The staging system of BRONJ was recently revised with the introduction of Sta ge 0, which includes bisphosphonate users with non-specific signs but without bone exposure in the oral cavity. According to the definition of BRONJ, it is not clear if Stage 0 patients are to be regarded as affected by non-exposed variant of osteonecrosis or only are at risk of developing BRONJ, given the fact that there is no evidence of exposed bone. The aim of the study is to asses the risk of patients with not-exposed BRONJ of advancing to higher stages. Our data suggest that Stage 0 patients are at high risk of advancing to established BRONJ. A significant percentage of cases initially complained of other oral manifestations than exposed bone. Necrotic bone is not the exclusive presenting sign and frequently occurs later following a non-exposed phase.
