Use of low-porosity PTFE (polytetrafluoroethylene) felt may be able to prevent intractable bleeding at the site of anastomosis in aortic repair operation.

Introduction Aortic dissection and thoracic aortic aneurysm is one of fatal diseases. The operative mortality rate is even high today. Then one of the most important things in aortic repair operation is the surgical technique. The use of PTFE felt for repair of diseased aorta has been reported by Cachera JP and others. When making anastomosis in aortic repair operation, soft-in-touch felt is often used. I was given the effect of firm-in-touch felt to prevent intractable bleeding and to reduce the mortality by Professor Matsumoto in University of Yamanashi. I have used the felt, Bard Low Porosity PTFE Felt (IMPRA, Inc., C. R. Bard, Inc.), for seven cases which were my first series between December 2006 and January 2008. Four of seven were performed total arch replacement, one of them was concomitant with Bentall operation, due to acute type A aortic dissection in three and chronic aortic dissection in one. And also two patients were performed ascending aorta replacement due to acute type A aortic dissection. One was performed thoracic descending aorta replacement. We will present the surgical procedure using low-porosity PTFE felt for aortic repair.
