The senior author designed a novel technique for cleft lip repair that he has utilized for the past 10 years. The technique begins like a conventional rotation advancement maneuver, but once adequate dissection permits a tension-free closure, the majority of the rotation flap, the columnellar flap and much of the advancement flap can be discarded. This closure that exerts minimal tension on the lip margin, anatomically aligns the orbicularis muscle, avoids significant nasal dissection, and permits a nearly straight line closure.
