Introduction: Coronary thrombosis leading to acute coronary syndrome (ACS) has a complex pathogenesis and in therecent few years the introduction of new intravascular imaging modality with high resolution has allowed tobetter characterize features of plaque instability. Furthermore, the combination of such important tool with theassessment of systemic biomarkers has opened the way to the identification of specific mechanisms ofinstability. In particular, plaque rupture and erosion seem to have different pathogenesis. In this article wedescribe the role of MPO, a marker of neutrophils activation, in coronary erosion and thrombosis.
