The target site for the radiofrequency (RF) ablation of ILVT can be determined using endocardial mapping by searching for the earliest presystolic Purkinje potential (PP), or a diastolic potential (DP) during tachycardia. We sought the significance of the recording PP and DP potentials as predictors of successful RF ablation. Methods: The study population consisted of 29 consecutive patients (22 men; age 27.97+-12.48) with ILVT undergoing RF ablation at our center. Results: The interval between the PP and the onset of QRS complex (PP-QRS) was 17.05+- 7.04 ms and the interval between DP and the onset of QRS complex (DP-QRS) was 89.44+-39.77 ms. In 18 (62.1%) patients, tachycardia termination was achieved with the RF delivery at the sites with a PP potential (with or without concomitant DP). In 9 (31.8%) patients, the target site of RF ablation was DP recording site.
