Ulcerative colitis and Crohn's disease are usually distinguished by a combination of clinical, histopathological and anatomical findings. Few studies have evaluated upper GI involvement in children with IBD.Thirty-two children with active IBD (24 with ulcerative colitis, 8 with Crohn's disease) with a mean age 12.1y and 13.8y, respectively, were studied. Upper GI endoscopy was performed and biopsy specimens were taken from duodenum, antrum, gastric body and oesophagus, and any visible mucosal lesions were evaluated by histology including Giemsa staining and urea test.Fifty-four per cent of patients with UC and 88% with CD were found to have inflammatory infiltration in gastric mucosa. In one UC patient, a polyp of the pylori was found, and another patient was additionally diagnosed with Menetrier disease. Only one patient with CD showed gastric mucosal epithelioid non-caseating granulomas. Microscopic inflammation in duodenal mucosa occurred in 33% of UC and 63% of CD patients. In one patient with CD, a type IE duodenal mucosa was diagnosed. All patients were Helicobacter pylori negative. Within our study group, only one CD patient (4%) was diagnosed with inflammatory lesions in the oesophagus.
