Anchorage control is one of the problems attracting the most attention in the field of orthodontic treatment with fixed appliances. This topic has become even more important due to the fact that an increasing number of adult patients present in the orthodontist's office. These patients want treatment that will improve aesthetics and function; they want the orthodontist to fix or reconstruct a deteriorated dentition. They want to stop the deterioration of their dentition. For many of these patients, the normal anchorage principles cannot be used, as a reduced number of teeth, a reduced perio-dontium and a lack of occlusion does not allow for conventional anchorage approaches. Still, many patients express a desire to maintain whatever is left of their dentition and to regain the original position of teeth that may have migrated spontaneously as a result of the extraction of one or more teeth or subsequent to a change in balance related to a progressing periodontal disease. This latter circumstance often will lead to extrusion in addition to marginal bone loss resulting in a deepening of the bite, which can worsen further following extraction of lateral teeth.
