Distribution of Thoracic Epidural Analgesia with Three Different Volumes of 1.5 Lidocaine

We evaluated the effect of three different volumes of 1.5% lidocaine on the spread of low thoracic epidural analgesia. On increasing the volume from 0.1 mL/kg to 0.3 mL/kg, an analgesic spread of only 3.84 was shown. The incidence of hypotension was higher with larger volumes. We concluded that 0.1 mL/kg of 1.5% lidocaine will establish relevant epidural analgesia preoperatively while achieving approximately eight analgesic segments without hypotension within 15 min after injection. The administration of 0.2 or 0.3 mL/kg of 1.5% lidocaine in low thoracic epidural space provided little advantage over 0.1 mL/kg.
