The Influence of Stereotactic Surgeries on Cognitive Functions in Parkinson's Disease

The study was undertaken to explore the effects of unilateral stereotactic Vim-thalamotomy (Vim-Th) and posteroventral pallidotomy (PVP) on cognitive functions of patients with Parkinson's disease (PD). Thirty-nine PD patients were examined with a neuropsychological test battery containing Trail Making Test, Stroop Colour Word Interference Test, Benton Visual Retention Test, Montgomery -^sAsberg Depression Rating Scale and Beck Depression Inventory two days before and three days after surgery in state "on". The neurological investigations by means of Unified Parkinson's Disease Scale (UPDRS), Hoehn & Yahr Staging of Parkinson's Disease and Schwab & England Activities of Daily Living were performed before and after surgery in state "on" and "off". Decreased depression level was obtained. No significant changes in cognitive functions except for improved processing speed within verbal working memory were observed. The results of neurological tests show significant improvement. The study indicates that unilateral PVP and Vim-Th influence postoperative emotional and neurological state but have little effect on postoperative cognitive state.
