Growing population of patients with end-stage coronary artery disease and ineffective routine treatment (PCI, CABG and maximal medical therapy) motivate scientists to develop new therapy. Angiogenesis can be induce by intramyocardial injection of autologous bone marrow stem cells. Our paper presents 2 year follow-up after delivery of stem cells together with transmyocardial laser revascularization (BMLR). Six Male patients (age 49 to 78 years) with end-stage coronary artery disease (CCS III/ IV) despite intensive pharmacological treatment and disqualified from prior CABG or PCI were included. Transmyocardial laser revascularization (TLMR) was performed with the Holmium:YAG laser at sites where CABG was impossible. Bone marrow concentrate was transplanted in the amount of about 1 ml per channel. No periprocedural complications occurred. We observed improvement in heart function stage, clinical condition ( from IV to I or II CCS) and quality of life and no major cardiac events after two years follow up. Moreover, we observed using cardiac magnetic resonace that BMLR therapy has potential to improve decreased myocardial function and may protect myocardium from the loss of viability and contractile function.
