The renovation of Spanish surgery began in the first years of the 18th century. It is not strange, therefore, to see how the findings were incorporated to the clinical teaching.One of the innovations insisted upon was "observaciones", observations, or charting the clinical history of the patients. This information was gathered and placed on the patient's headboard and included all treatment details to be later reported.The existence of the Files of the Royal School of Surgery of Cadiz (1748-1836) containing 306 "observaciones" (observations) has induced us to elaborate in this paper. Mainly first and second surgeons did these "observaciones" from the different Army sites as well as the teachers of the Royal School of Surgery. Among these "observaciones", we find one dated 1787 by Pascual De Vega concerning the treatment of a person who drowned in Barcelona's bay [1].
