An adaptable health screening questionnaire

A computer-based lifestyle questionnaire has been developed at Canterbury University for general practitioners wishing to improve the preventative side of their practice. Its major innovative feature is the ability to allow individual doctors to modify the questionnaire to their requirements. The questionnaire itself is based on an expert systems approach, which allows selective progress through the questionnaire structure, based on responses to previous questions. The menu-driven user interface encourages use of the questionnaire by a wide spectrum of patients.
