Robotic testing can evaluate which surgical techniques return joint kinematics closest to native. Larger differences between native and deficient provide a larger range in which to evaluate ACL reconstructions. This is a key property of a more robust test. For flexion angles prior to the shift, simulated pivot shift showed ATT changes similar to the anterior drawer (p=.268) and significantly larger than the rotary loads test (p<.001). In addition, the simulated pivot shift showed significantly larger ER differences (p<.001) which suggests it can quantify rotary instability, unlike rotary loads. Based on the results of this work, the simulated pivot shift should undergo further refinement and eventual validation to serve as a better tool than rotary loads for evaluating ACL reconstruction techniques.
