What problems arise during a "real life" hazard analysis? The BB21 Signalling System is being developed for the Nederlandse Spoorwegen (the Dutch railways) under the supervision of Prorail-Railinfrabeheer, the Dutch rail infrastructure provider. Instead of signals along the track, BB21 sends the signals by radio to a cab display in front of the train driver. Taking the BB21 Signalling System into use requires the approval of the Dutch railway safety authority. The hazard analysis technique used for the BB21 Signalling System is based on Railtrack UK's "Yellow Book". The hazards to be analysed were identified using the process described in part Ⅰ of this paper (Step 1 in the Yellow Book). Part Ⅱ now describes the hazard analysis process itself and shows how the Yellow Book guidelines were adapted to the requirements of BB21. Where possible, the paper provides guidelines based on the authors' experience.
