Bioadhesive buccal tablet formulation of methylprednisolone for post-operative pain and swelling in oral surgery

The surgical removal of impacted thrid molars can result in considerable pain, swelling and trismus~1. Surgical removal of lower third molars is the most common measure in oral surgery and serves as an excellent model for studying pain~2-4. The ideal agent for use after third molar surgery should alleviate pain, reduce swelling and trismus to a minimum, promote healing and have no unwanted effects. For relief of pain, analgesics are the obvious choice. Where possible, an analgesic with additional anti-inflammatory properties should be used. Several studies have been undertaken to evalaute the efficiency of corticosteroids to prevent undesirable postoperative reactions. When properly used, corticosteroids are a valuable asset in the treatment of inflammation; however, the patient's well-being may be compromised if steroids are not appropriately given. It is apparent that corticosteroids must not be used haphazardly; because of their unwanted side effects, therefore the risk/benefit ratio of their use must be carefully considered before initiation of therapy. Generally, topical use of corticosteroids is preferred due to the decreased side effects~5.
