The purpose of this study is to establish a personalized hierarchical diagnosis and treatment model for the chronic diseases based on the application of the clinical pathway technology, so as to provide a basis for the human body model in the state control research. The model put forward in this paper is based on the exercise status and the eating habits (including the intake of salt, cereals, vegetables, fruits, meat and poultry, eggs, fish and shrimps, beans, dairy products, oils, animal internal organs and so on). The drinking, smoking and so on are taken as the input amount, and the output amount includes the systolic blood pressure, diastolic blood pressure, blood sugar, heart rate, BMI and other physiological parameters which are closely related to the chronic diseases. In addition, residual analysis is carried out the test the reliability of the model established in this paper. Through the trial of 13 volunteers, the accordance rate of the measured parameters and the estimated parameters in 9 cases is more than 80%. The research results show that the evaluation and optimization method for the personalized chronic disease risk factors based on the clinical pathway put forward in this paper is feasible in general, which has provided a basis for the design of personalized risk factor control strategies accordingly.
