A micrometer-scale or nanometer-scale device is described that can include integrated circuit logic based on sub-10 nm SIA transistor nodes, a sensing subsystem, a decision subsystem, a validation subsystem, and a power harvesting system. The sensing subsystem may identify pathogenic entities including disease-associated cells (eg, cancer cells, autoimmune cells, or pathological microorganisms) or viruses. The sensing subsystem may include at least one pad comprised of an electrically conductive material and a linker attached to the at least one pad. Each linker may also be attached to a targeting agent (eg, an antibody fragment) or a reference agent. Logic circuitry that, upon binding of a target agent to a subject of interest, processes binding event information, determines whether the subject is disease-associated (eg, a disease-associated cell or virus), and requires delivery of a therapeutic agent or other therapeutic agent information is transmitted through
