A surgical instrument for performing operations, especially operations in the region above the pubic area, consists of a trocar with a shaft (3) which has a pointed end (3'). The trocar is enclosed in a sheath (7) which has an axial through hole (14) to receive the trocar and is mounted in a housing (2). The trocar housing (2) is provided with a sealing element (11) which seals the end of the sheath (7) when the trocar has been removed from the sheath. The sealing element is positioned in the region (8) which is axially at a distance from the shaft tip (3').
