The sheet contains several printed answers for each question and with a box alongside each to be crossed or filled out. The boxes are contained in a recess one above the other. It comprises two duplicating sheets (12, 13) fixed together. The recess in the first sheet containing the boxes (6) is formed by a detachable strip (4, 5). The recess (18, 19) thus exposed behind it contains only boxes (16) in line with the correct answers. Where the complete sheet (1) is printed only on one side, the individual sheets can be formed by top and carbon copies. ADVANTAGE - Simple evaluation without the need for auxiliary equipment.
