PURPOSE:To surely register previous illness data to the health care card of a patient without burden to a doctor. CONSTITUTION:When the doctor registers medical history data of the patient to a health care card 17 by a reader/writer 15, a processing part 11 displays contents of this medical history information on a touch panel display device 12. If contents of, a treatment condition item of this displayed medical history information shows that treatment is completed, the processing part 11 writes the name of the disease of medical history information in the health care card 17 of the patient as a previous illness of the patient independently of registeration of medical history information. Then, the previous illness as important materials for doctor's medical examination to the patient is automatically recorded in the health care card of the patient without requiring a special operation. Thus, the previous illness registering work is not a burden to the busy doctor, and he is prevented from forgetting to perform this work.
