PURPOSE: To much more attain the miniaturzing of an optical information read- out device, etc., with the integrations of a special form. CONSTITUTION: A read-out sensor module 14 as a lens integrating elements is supported to a GRIN lenses 20 while the read-out sensor 26 being an photographing element turns the light receiving plane with respect to the lens 20 allowing a light to be made incident from the incident plane 20c being the end plane of one side and focusing the light on the light receiving end plane 20d being the end plane of the other side. Moreover, electronic circuits 22, 24 are formed on surfaces other than both end planes 20c, 20d. Consequently, the module 14 in itself and also a scanning part in which the module 14 is used or also the whole of a bar-code reader in which the module 14 is included are miniaturized much more. Further, the designing of the scanning part and the producing of the scanning part and the module 14 in itself are made efficient.
