The disclosed electronic survey device (100) is a portable, hand held unit particularly adapted for use in restaurants or other food service facilities, but also usable in hotel, retail, or many other environments where customer satisfaction survey information is desired to be gathered. The device presents a series of questions, records the answers, and then stores the complete survey response. Periodically, it can be connected to a host computer (120) for upload of the responses, or download of new questions. Alternatively, the device can also download personalization information which allows questions and prompts to the tailored to the specific customer using the device. The preferred form of the device is to incorporate it into the check book (102) is traditionally used to present the customer's check.
