PURPOSE: A form for a conduit on a roadwork is provided to install and dismantle a form simply and rapidly with simple structure, to shorten a period of construction and to reduce the whole construction expenses by recycling a form. CONSTITUTION: The form for a conduit on a roadwork comprises: an outer plate erected on both sides of the surface of an excavated road(300) by a fixture(160); an upper holder(110) fastened on the upper side of the outer plate; an inner plate(130) erected together with the upper holder(110) to the inside of the outer plate(120); a lower holder(140) installed on the lower side of the inner plate(130) to keep a space of the inner plate(130); and a floor plate installed on the lower end of the inner plate(130).
