PPROBLEM TO BE SOLVED: To manage users and helpers by entering day books as images. PSOLUTION: The report management system comprises an image entry sub-system provided with a helper's day book input means and a contract assessment table input means by which a helper's day book and a contract assessment table are optically read out, the read results are converted into digital information and the digital information is recorded in an integrated database and a user assessment history sub-system for preparing and updating a user assessment history file. The system comprises also a team care management sub-system for generating a countermeasure method and warning to care team members on the basis of the updated user assessment history file when user's condition exceeds a fixed range, a visiting care plan entry sub-system for planning visiting care on the basis of the user's assessment history file and the integrated database, and a sales management sub-system for calculating medical treatment fees on the basis of the visiting care plan and working results.
