PROBLEM TO BE SOLVED: To determine whether or not an injury/disease severity is equal to or greater than a prescribed level even when information is small in amount and poor in accuracy.SOLUTION: An injury/disease severity identification device includes a display control part 101 to display an input screen including input items to be used for identification of an injury/disease severity, an input reception part 102 to receive an input of information in an input item, a required item determination part 103 to determine whether or not required items among the input items has been input, a sever injury/disease identification part 104 to identify whether or not the injury/disease severity is equal to or greater than a prescribed level on the basis of the information input in the required items, an identification propriety determination part 105 to determine whether the injury/disease severity can be identified on the basis of the information input in the input items if the sever injury/disease identification part 104 determined that the injury/disease severity is less than the prescribed level when all of the required items have been input, a mild injury/disease identification part 106 to identify the injury/disease severity on the basis of the information input in the input items if the severity identification part 104 determined that the injury/disease severity is less than the prescribed level when all of the required items have been input.
