The present invention relates to a hybrid light collection module which integrates a dynamic element with a passive and flat optical waveguide element. The hybrid light collection module includes multiple optical waveguides. Multiple trenches are formed on each of the optical waveguide areas from the upper part to the core in the length direction of the optical waveguides. Each of the trenches includes: the optical waveguide element which is injected with an index matching material having a refractive index larger than that of the core; and the dynamic element which is mounted on the upper part of an area with the trenches. The trenches on the center are longer, and the width of each trench corresponds to the width of the optical waveguide core. As the light signals passing through the flat optical guide element goes toward the dynamic element from the area with the trenches, a part of the light signals are branched to the dynamic element through the trenches.
