PROBLEM TO BE SOLVED: To provide a medical record creation support system which solves complexity of operability, and lightens burden of data input for creating medical record for doctors and medical institution or the like.SOLUTION: There is provided a medical record creation support system in which medical institution terminal comprises: area information accepting means for accepting information regarding an area that constitutes an organism in response to an input operation of a user; symptom specific information transmission means for transmitting information regarding identification of a symptom; symptom candidate receiving means for receiving a symptom candidate; symptom selection accepting means for accepting selection of a symptom from received symptom candidate in response to an input operation of a user; and progress information accepting means for accepting progress information regarding a chief complaint in response to the input operation of the user, and the server device comprises: symptom specific information receiving means for receiving symptom specific information; symptom candidate extracting means for extracting a symptom corresponding to a received area as a symptom candidate; and symptom candidate transmission means for transmitting an extracted symptom candidate.
