The present invention provides a surface-modified zirconium-based implant including an implant substrate having a hydroxyl group introduced into its surface and a linker. The present invention also provides a zirconium-based implant for inducing or promoting bone formation comprising a growth and differentiation factor (GDF) immobilized on a linker of a surface-modified implant. The surface-modified zirconium-based implant according to the present invention can be manufactured through a simple click reaction, which is simple in process and high in yield. The linker formed on the implant substrate through a click reaction can stably and easily immobilize heparin or growth factor and differentiation factor (GDF) on the implant. In addition, the zirconium-based implant for inducing or promoting bone formation according to the present invention slowly dissolves the growth and differentiation factor (GDF) for a long period of time, thereby solving the problem caused by premature release of the bone-inducing factor, Thereby improving bone formation and osseointegration of the implant.
