PROBLEM TO BE SOLVED: To provide a merchandise delivery confirmation slip creation system, a merchandise delivery confirmation slip creation program, and a merchandise delivery confirmation slip that can reduce labors for both a customer and a shop, and can prevent an increase in scale and cost of the system.SOLUTION: A merchandise delivery confirmation slip creation system comprises: an acquisition part 122 that acquires information to be described on a merchandise delivery confirmation slip of merchandise to be delivered to a customer; a code symbol creation part 126 that creates a two-dimensional code obtained by encoding delivery state and location information that is the URL of a web page showing the delivery state of the merchandise; an image creation part 127 that creates image data of the merchandise delivery confirmation slip describing the information acquired by the acquisition part 122 and the two-dimensional code; and an image formation request part 128 that requests an image forming apparatus to form, on a sheet, an image based on the image data created by the image creation part 127.SELECTED DRAWING: Figure 9
