The present invention relates to a system and a method to provide a healthcare service reflecting a smart health management strategy. The present invention includes: a self-evaluation performing part providing a self-evaluation interface to a patient to evaluate the life quality and health condition of the patient and then receiving evaluation data from the patient, and providing an evaluation result of analyzing the evaluation data to the patient; a health information providing part providing health information to the patient based on the evaluation resu an execution planning part receiving a health management plan selected by the patient, and providing a schedule management interface to the patient to manage behavior included in the health management plan; and a monitoring part monitoring whether the health management plan is fulfilled or not.
