In the present invention, the direction of a flow of air discharged from an air purification device (2) provided in a ceiling surface (13) is inclined with respect to the direction of an operating table (14), and the flow of air is constant. In addition, in the present invention a structure is provided whereby several small lighting units are arranged independently in a distributed manner while maintaining their illumination capability as a surgical lighting fixture, thereby enabling the air flow to pass through an aperture-type shadowless lamp (3) in which an air gap of no less than 50% is provided in the interior of the lighting fixture. Thus, the surgical field above the operating table (14) can be purified efficiently with a low number of ventilation operations, so bacterial contamination of the air during an operation can be prevented effectively and with reduced energy and a low cost.
