Auxiliary positioning device for surgical interventions comprising a support system (10) and a template (12), the support system (10) being composed of a support plate (16), provided with a recess (18), and supports (20) by means of which the support plate (16) can be fixed in a cranial bone (14) above an operative field for surgical intervention, and the template (12) being composed of a template plate ( 24) with a guide hole (26), said plate being able to be attached without play to the support plate (16), the template plate (24) being composed of a blank in which the guide hole (26) is arranged and guides individually by removing material from the blank according to predetermined coordinates, and coinciding with the longitudinal central axis (36) of the guide hole (26), in the assembled state of the support system (10) and of the template (12), and the support system (10), set at the cranial bone (14), with a path for access to the operative field for surgical intervention, characterized in that the longitudinal central axis (36) of the guide hole (26) is arranged eccentrically with respect to the template plate (24 ), presenting an inclination of its longitudinal central axis (36) that deviates from the vertical of the surface of the template plate (24), and dimensioning the recess in the support plate (16) so that the guide hole (26) on the template plate (24) is not covered by the support plate (16) even in case of a maximum eccentric arrangement.
