The instrument has a force transmitting member (44) moveably arranged in a tubular shaft (42), and tool elements (30, 32) moveably arranged at a distal end of the shaft. The force transmitting member is coupled moveably with a grip portion for transferring an operating force of the grip portion to the shaft. An insulation shaft positioning device holds an insulation shaft (108) in a distal position relative to the shaft. The positioning device is formed such that no force is exerted on the insulation shaft if the insulation shaft occupies the distal position relative to the shaft.
