The instrument has a tubular shaft (12) defining a longitudinal axis (20) and surrounding a support element. A tool element (18) and the shaft include respective actuating sections. The shaft and the support element are displaceably supported parallel to the axis. The actuating sections are formed and engaged to each other such that the tool element is movable and positively driven from one tool element position to another position and vice versa due to a relative movement of the shaft and the support element in distal and proximal directions, respectively.
