The instrument (10) has a carrier unit (14), and a tool unit (18) that is movably supported at a distal end of the carrier unit. Actuating sections (128, 134) are provided at the tool unit and at a shaft, respectively. The sections are designed such that the sections engage and act together such that the tool unit is movable in a positively driven and exclusive manner from one tool unit position into another tool unit position and vice versa due to relative movement of the shaft and the carrier unit in distal and proximal directions, respectively.
