A surgical instrument (100, 110) is provided with a medical treatment tool (1a, 101a) and an attached device (6, 106, 117) to which the medical treatment tool (1a, 101a) is mounted. The medical treatment tool (1a, 101a) has: a distal-end part (20, 120) capable of operating with multiple degrees of freedom; a slender element (14, 114) which is a wire or cable for manipulating the distal-end part (20, 120); and a shaft (12, 112) for accommodating the slender element (14, 114), the distal-end part (20, 120) being connected to the shaft (12, 112). The medical treatment tool (1a, 101a) furthermore has an engaged member (13, 113) connected to the slender element (14, 114) at the opposite side from the distal-end part (20, 120) in the longitudinal direction of the shaft (12, 112). The attached device (6, 106, 117) has: an engaging member (84, 184) driven by a drive source (81), the engaging member (84, 184) engaging with the engaged member (13, 113); and a release mechanism (73, 173) for making it possible to disengage the engaging member (84, 184) and the engaged member (13, 113) from each other.
