#$%^&*AU2019101484A420200123.pdf#####Abstract The Community Restoration Transitional Support Unit (CRTS) program provides short-term transitional support care that seeks to optimise the participants functioning ability to provide supported independence living service provision to develop the capacity building for the people with disability, after a hospital and aged care stay. the CRTS program is person centred goal focussed approach to ease the participant's crisis situation by providing immediate specialists' accommodation 24/7 support time-limited and therapeutic-focused in order to restore the participants into general community through the specialists' service coordination after CRTS living skills assessment evaluation outcome permanent services. It provides NDIS participants with a CRTS interim crisis support package as short-term transition support care which 6-month time bond services that includes low intensity care therapy such as physiotherapy and occupational therapy, as well as specialists service coordination clinics social work, nursing support or personal care assistance It seeks to enable people with disability to restore in community while living in CRTS unit after a hospital stay rather than enter residential ager care facility prematurely.
