The present invention relates to liposomal nanocarrier delivery systems for targeting active CD44 molecules, methods of making the same, and uses thereof. The surface of the liposome is partially modified by a targeting ligand, wherein the targeting ligand is a ligand that can be specifically combined with an active CD44 molecule. The liposomal nanocarrier delivery system can be used to diagnose, prevent, and treat fragile plaques or diseases associated with fragile plaques.
