Natural Orifice Transluminal Endoscopic Surgery (N.O.T.E.S.) has been seen as a 'revolution' in surgery although to date experience has mostly been confined to experimental models with the aim of replicating procedures already adeptly performed at laparoscopy using natural orifice access. This is understandable, for as much as certain commentators want evidence that N.O.T.E.S. procedures are better than standard techniques, the onus must be to first show that transluminal abdominal surgery is consistent with safe operative performance. Only then can subsequent experience be expected to discriminate comfort and acceptability (if not preference) of this new approach.
