Techniques for advancing the base of tongue and modifying the hypopharyngeal airway in obstructive sleep apnea have undergone various modifications since they were first described by Powell and Riley. The purpose of this article is to describe an advancement procedure for the genioglossus muscle utilizing a three-piece anterior visor osteotomy of the mandible as an adjunctive procedure for improving the hypopharyngeal airway in obstructive sleep apnea.
