Cerebral ischaemia following cardiopulmonary resuscitation in pigs

We read with interest the article by Ristagno et al.,1 evaluating the cerebral cortical perfusion following cardiopulmonary resuscitation in healthy pigs. Though the experimental approach is ambitious, there are some points we wish to comment on.Firstly, the authors found an increased tissue partial pressure of carbon dioxide (PCO_2) and conclude this to indicate cerebral ischaemia following cardiac arrest and cardiopulmonary resuscitation. However, the authors did not present any data on systemic pCO_2. We agree that tissue pCO_2 may serve as a useful quantitative surrogate variable of the metabolic consequences of Low-flow states with only small changes in systemic pCO_2.
