Although the exudative form occurs in only 8% of eyes with age-related macular degeneration, it is re-sponsible for 85% of severe vision loss. Macular translocation has been used to treat age-related macular degeneration with some success. Methods include full macular translocation (FMT) and partial translocation, with or without chorioscleral shortening maneuvers. Each of these techniques involves the creation of an artificial retinal detachment by injection of a subretinal infusate in the periphery or posterior pole. Achieving a consistent uniform detachment is often the most time-consuming step in these procedures. This is especially true for slightly younger patients who require induction of a posterior vitreous detachment at the time of surgery. These patients tend to have a neurosensory retina that is more adherent to the retinal pigment epithelium (RPE). We describe a combination method of detaching the retina in preparation for FMT for the treatment of subfoveal choroidal neovascularization dueto age-related macular degeneration. Theoretical implications for achieving a rapid total retinal detachment are also described.
