An Article1 and a Review2 on rapid-eye-movement (REM) sleep behaviour disorder (RBD) in this issue of The Lancet Neurology are good examples of the unanticipated clinical development of a simple isolated observation: that the absence of atonia during REM sleep could result in dramatic dream-enacting behaviour. After the prediction of the existence of RBD on the basis of animal experiments, and its formal identification in man in 1986,3 it soon became apparent that over time increasing numbers of patients with RBD were developing neurodegenerative disorders—particularly the synucleinopathies Parkinson's disease (PD), dementia with Lewy bodies (DLB), or multiple system atrophy.
