Objective information on the basis of controlled studies between medical civilian action projects (MEDCAPS) and alternative forms of humanitarian medical assistance is lacking but essential for formulating guidelines and enforcing medical rules of engagement in Iraq, Afghanistan, and future counterin-surgency operations. This editorial suggests a more informed, legitimate, and lasting approach over what has become the most popular form of medical assistance in Iraq
