Towards a tighter link between supervision and trainee ability.

Generations ago, the experience of independent clinical practice was embedded in the process of postgraduate medical education. After hours and on weekends, the attending staff would generally leave the hospital and medical trainees would be left to run the wards, the emergency rooms and the operating suites. In this 'do or die' environment, trainees were required to function independently, and they learned clinical medicine through the practical experience of independent clinical decision making.
