Combining carbamazepine, neuroleptics and acetylcholinesterase inhibitors with methylphenidate only reduces adverse side effects, but is less effective than a combination with atomoxetine.

Methylphenidate, a dopamine reuptake inhibitor, has been developed for Attention Deficit Hyperactivity Disorder (ADHD). Emotional lability, including suicidality, as well as aggression, may also be a problem limitating Methylphenidate's and also Ato-moxetines benefit ([4]). On the one hand, acetylcholinesterase inhibitor's cognition enhancing properties suggest, that they may help to reduce co-administered Methylphenidate (MPH). On the other hand, methylphenidate's anticholinergic side effects [6], mainly xerostomia, lead to the hypothesis, that Acetylcholinesterase inhibitors may reduce its dosage, have neuroprotective effects, and also improve its efficacy. Atomoxetine (ATX), a noradrenalineorepinephrine reuptake inhibitor, has also been developed for Attention Deficit Hyperactivity Disorder (ADHD). Atomoxetine's mode of action and especially its pro-cholinergic property [5], even if anticholinergic side effects like xerostomia have been reported, leads to the first hypothesis, that acetylcholinesterase inhibitors may reduce its dosage, have neuroprotective effects, and also improve its efficacy.
