Retained intraocular foreign bodies (IOFB) account for a small but significant number of ocular trauma cases. To our knowledge, an IOFB retained for more than 60 years has not yet been reported. One such case is reported here.A 74-year-old man who complained of progressive blurry vision in the left eye for 10 years was admitted. He described being accidentally struck in his left eye when a blacksmith was hammering an object using an iron hammer 64 years ago. He did not attend hospital at the time and his eye settled after a few days. On examination, visual acuity was 6/15 in the right eye and counting fingers in the left eye. Anterior segment examination of the left eye showed a clear cornea with a small stromal scar outside the visual axis, a small defect in the iris at 4 o'clock, and a brown opaque lens (Fig. 1). There was no heterochromia. The intraocular pressure (IOP) was 16 mm Hg and 15 mm Hg in his right and left eye, respectively. Pupil reactions and gonioscopy were normal. Fundus could not be seen because of the opacity of the lens. B-scan ultrasonography showed a highly echogenic mass located in front of the retina (Fig. 2). A plain x-ray of orbits suggested the presence of a high-density IOFB.
