Acute generalized exanthematous pustulosis induced by a topical agent: 2-chlorobenzylidene malonitrile (CS) gas.

Madam, A 37-year-old man presented with a 2-day history of a tender, pustular rash, affecting the face, chest, arms and anterior thighs. Twelve days before presentation to the dermatology unit he had been sprayed with 2-chlorobenzylidene malonitrile (CS) gas. At that time, he had developed a severe blistering rash on the sites in contact with the CS gas. He was admitted to the plastic surgery department where he received intensive topical therapy (silver sulphadiazine cream to the body, liquid soft paraffin and mupirocin ointment to the face along with a combination of paraffin gauze, hydrocolloid and lipidocolloid dressings). During this initial admission, he also received paracetamol, codeine phosphate, morphine sulphate solution, hydroxyzine and intramuscular tetanus vaccination.
