No one can be in any doubt that current NHS provision is unfit for purpose, as evidenced by the continuing media reports of struggling A&E departments and, more recently, cancelled booked operations together with central government bailouts of overspent NHS trusts. The recent publication of the Five Year Forward View (NHS England, 2014) sets forward the reasons why an NHS designed to meet the health-care needs of the population in 1948 is no longer appropriate for an ageing population with complex health problems, some of which arise from lifestyle choices.
