Edwards and Sines provide an understanding of the process whereby emergency nurses modify their appraisal of patients from initial impression to final evaluation through triage assessment. The study found that nurses incorporated multiple inferences into triage assessments based on patient and caregiver presentation. The findings can help nurses to recognise the effects of their impressions and biases on triage. Outstanding questions include how the findings may affect objective triage scales, such as the .Canadian Triage and Acuity Scale (CTAS),1 and how the concept of patient-centred care fits within nursing activities. The goal of the CTAS is to create objective categories that identify medical needs and establish safe wait times. The presenting complaint is the primary determinant,guided by modifiers such as vital signs, pain, and chronicity.' The study by Edwards and Sines calls into question the effectiveness of the objective CTAS. Nurses' reasoning and judgement are strengthened by knowledge processes that incorporate systematic assessment approaches. How nurses' impressions influence objective triage is unclear.
